Group Life Insurance can be purchased by companies, associations, unions, and other entities that have an insurable group. The insured individuals must have a connection with the policyholder (the contracting company or entity), such as an employment or membership relationship.

1. Written complaint document filed with the Carrier, such as a "Baggage Irregularity Report," "PIR - Property Irregularity Report," or equivalent.
2. Police Report.
3. Original baggage tickets for all checked items.
4. Certificate or statement from the Carrier accepting responsibility for the lost baggage and/or compensation receipt issued by the Carrier, with a settlement agreement.
5. Compensation receipt from the transport company signed by the claimant.
6. List of items contained in the baggage with their respective values.
7. Proof of notification of the lost baggage.

1. Invoice for the repair of the damaged luggage or a report stating that repair is not feasible, along with the invoice for the purchase of a new luggage.
2. Original baggage ticket for all checked items.
3. Description of the damaged item(s) due to a covered incident.
