- Insurance Policy or Travel Card.
- Brazilian Insured: ID, CPF, and proof of residence.
- Foreign Insured: Copy of passport.
- Driver’s License, required for traffic accidents.
- Police Report if applicable.
- Insured's Bank Details.
Documents for each coverage
For each coverage, we list the mandatory and optional documents that help in the analysis.
- Death Certificate.
- In case of accident or illness, complete medical documentation.
- Medical report justifying the insured's hospitalization on the scheduled travel date.
- For interruption due to a family member, all documents proving that the person was a family member of the insured will be required.
- Letter from the operator/agency detailing the penalties and costs to be borne by the insured.
- Insurance Ticket or Travel Card.
- Documents proving the amounts paid.
- Proof of the penalty amounts retained in case of trip interruption, including enrollment fees for the exchange course.
- Service contract from the travel organizers.
- Technical report and/or documentation that proves the reason for the trip interruption.
- Requests and results of all exams performed, along with their respective reports.
- In case of a diagnosis of serious illness: complete medical documentation.
- In case of hospitalization of the spouse, parent(s), sibling(s), parent(s)-in-law, or child(ren) of the insured: complete medical documentation.
- In the event of a judicial summons for the insured: copy of the summons and the judicial process.
- In the case of a quarantine decree imposed on the insured: copy of the declaration from the health authority.
- Documentation of the beneficiary(ies):
- Spouse of the insured: marriage certificate and spouse's ID card.
- Family member of the insured: annotation in the work card or proof of dependents in the social security system and ID card of the family member.
- Child of the insured: birth certificate.
- Police Report.
- Death Certificate.
- In case of accident or illness, complete medical documentation.
- Letter from the operator/agency detailing the penalties and costs to be borne by the insured.
- Ticket or Travel Card.
- Copy of the Insurance Ticket.
- Proof of amounts paid.
- Proof of penalty amounts retained in case of trip interruption.
- Service provision contract from the travel organizers.
- Technical report and/or documentation proving the reason for trip interruption according to the covered events.
- For family member reasons, supporting documents.
- Requests and results of all exams performed, with their respective reports.
- In case of a serious illness diagnosis: copy of all exams performed and detailed medical report.
- In case of hospitalization of the spouse, parent(s), sibling(s), in-law(s), or child(ren) of the insured: detailed medical report, results of performed exams, and documents proving the relationship.
- In case of a judicial summons to the insured: copy of the summons and the judicial process.
- In case of quarantine imposed on the insured: copy of the health authority's declaration.
- Death Certificate of the person whose death motivated the trip cancellation, if applicable.
- Medical report detailing the diagnosis and treatment of the person whose illness caused the trip cancellation.
- Proof of family relationship of the insured with the beneficiary or the person who motivated the trip cancellation, such as marriage or birth certificate.
- Weather Report or equivalent document from the competent authority that caused the trip cancellation, if applicable.
- Judicial notification.
- Declaration from the competent health authority determining the mandatory quarantine for the insured, if applicable.
- Service contracts for the trip, which must provide for penalties in case of cancellation.
- Proof of payment for all travel services purchased, including exchange program registration fees.
- Beneficiary Documentation:
- Spouse of the insured: marriage certificate and spouse's ID.
- Family member of the insured: annotation in the Work Card or proof of dependents in the INSS and ID of the family member.
- Child of the insured: birth certificate.
- Ticket or Travel Card.
- Insurance Ticket.
- Proof of amounts paid.
- Proof of penalties withheld in case of cancellation.
- Service contract from the travel organizers.
- Technical report and/or documentation proving the reason for cancellation according to covered events.
- Documents proving that the person was a family member of the insured.
- Medical report justifying the reason and confirming the hospitalization of the insured on the scheduled date of travel.
- Requests and results of all examinations performed, with their respective reports.
- Medical report informing the injury suffered and the treatment adopted, proving the impossibility of locomotion.
- Presentation of documentation proving the kinship.
- Death certificate of the person whose death motivated the cancellation of the trip, if applicable.
- In case of diagnosis of serious illness: copy of all examinations performed and medical report with the description and date of the first diagnosis of the illness affecting the insured.
- In case of hospitalization of the spouse, parent(s), father-in-law(s), sibling(s) or child(ren) of the insured: medical report that determined the hospitalization, requests and results of all examinations performed, with their respective reports, medical records, and supporting documents of the kinship condition.
- In case of judicial summons of the insured: copy of the summons and judicial process.
- In case of quarantine imposed on the insured: copy of the declaration from the health authority.
- Documentation of the Beneficiary(ies):
- Spouse of the insured: marriage certificate and identity card of the spouse.
- Family member of the insured: annotation in the work card or proof of dependents in the INSS and identity card of the family member.
- Child of the insured: birth certificate.
- Stamped passport proving the deportation.
- Proof of expenses to be reimbursed.
- Travel ticket or card.
- Insurance ticket.
- Documents proving the amounts paid.
- Proof of penalty amounts withheld in case of cancellation.
- Service provision contract from travel organizers, which must provide for penalties in case of cancellation, as per legal requirements.
- Technical report and/or documentation proving the reason for cancellation according to the covered events.
- Presentation of documentation proving the family relationship.
- In case of damage caused by fire, theft, or force of nature in the residential or commercial property used regularly by the insured: police authority reports and proof of property ownership or possession.
- In case of the insured's summons to participate as a member in the official elections of their home country: copy of the summons notice with date and location details.
- In case of complications during pregnancy of the insured or the insured's spouse: copy of the medical report, copy of medical examinations performed, and proof of the spouse's condition if applicable.
- In case of involuntary unemployment of the insured: copy of the employer's notification and a copy of the employment card.
- In case of the insured's summons for assuming a position in public service or with a new employer: copy of the communication directed to the insured, a copy of the Official Journal with the appointment or a copy of the Professional Card, as applicable.
- In case of the insured's work location transfer: copy of current proof of residence, copy of the employer's notice indicating the destination or region of the new residence, and a copy of the professional card showing the transfer record.
- In case of denied passport or visa: copy of the visa or passport denial notification.
- Pregnancy occurring after purchasing insurance, and being unable to travel during the trip due to the advancement of pregnancy: proof and medical report.
- In case of document theft: police report, copy of new documents such as a visa if applicable.
- In case of vehicle damage or accident: police report with a photo of the vehicle damage.
- For all reasons: presentation of supporting documents for the reason for cancellation is required.
- Documentation of the beneficiaries:
- Spouse of the insured: marriage certificate and identity card of the spouse.
- If the beneficiary is a family member of the insured: annotation in the Work Card or proof of Dependents in INSS and identity card of the family member.
- If the beneficiary is the child of the insured: birth certificate.
- Travel ticket or card.
- Insurance ticket.
- Documents proving the amounts paid.
- Proof of penalty amounts withheld in case of cancellation.
- Service provision contract from travel organizers.
- Technical report and/or documentation proving the reason for cancellation according to the covered events.
- For cancellations due to a family member, all documents proving that the person was a family member of the insured will be required.
- Declaration/Report from the attending physician, justifying the reason and confirming the insured's hospitalization on the scheduled date for the trip, signed and notarized.
- Results of examinations performed and declaration/Report from the attending physician, informing the occurred injury and proof of inability to move, signed and notarized.
- Presentation of documentation proving the family relationship.
- Death certificate.
- In case of diagnosis of a serious illness: copy of all examinations performed and medical report with the description and date of the first diagnosis of the illness affecting the insured.
- In case of hospitalization of the spouse, parents, siblings, in-laws, or children of the insured: copy of the medical report that determined the hospitalization, copy of all examinations performed, copy of the medical record, and copy of the documents proving the family relationship.
- In case of judicial summons of the insured: copy of the summons and the judicial process.
- In case of quarantine imposed on the insured: copy of the declaration from the health authority.
- In case of damages caused by fire, theft, or force of nature in the residential or commercial property regularly used by the insured: police authority reports and proof of property ownership or possession.
- In case of summons of the insured to participate as a member in the official elections of their home country: copy of the summons notice with date and location details.
- In case of judicial summons of the insured as a guardian in the adoption process of minors: copy of the summons and the judicial process.
- In case of complications during the pregnancy of the insured or the insured's spouse: copy of the medical report, copy of the medical examinations performed, and proof of the spouse's condition if applicable.
- In case of involuntary unemployment of the insured: copy of the employer's notification and a copy of the employment card.
- In case of summons of the insured for organ transplant: copy of the summons and medical description, copy of examinations performed and the hospital medical record.
- In case of summons of the insured for assuming a position in public service or with a new employer: copy of the communication directed to the insured, copy of the Official Journal with the appointment of the insured or copy of the Professional Card, as applicable.
- In case of work location transfer of the insured: copy of current proof of residence, copy of the employer's notice indicating the destination or region of the new residence, and copy of the professional card showing the transfer record.
- In case of judicial summons of the insured for divorce proceedings: copy of the summons and copy of the judicial process.
- In case of denied passport or visa: copy of the visa or passport denial notification.
- Pregnancy occurring after purchasing insurance, and being unable to travel during the trip due to the advancement of pregnancy: proof and medical report.
- In case of document theft: police report, copy of new documents such as a visa if applicable.
- In case of vehicle damage or accident: police report with a photo of the vehicle damage.
- For all reasons: supporting documents for the reason for trip cancellation.
- Documentation of the beneficiaries:
- Spouse of the insured: marriage certificate and identity card of the spouse.
- Family member of the insured: annotation in the Work Card or proof of Dependents in INSS and identity card of the family member.
- Child of the insured: birth certificate.
