CNH (National Driver's License) - Must be within the validity period.
Bank DeBank Details - (Bank name, agency, and account) must be in the name of the insured, be a personal account, and be registered in the IZA app.tails
Selfie video of no more than 1 minute recounting the events on the day of the accident, including the date, time, and location.
Police ReportPolice Report - It can be filed at the police station after the accident (by declarative act). It is mandatory for traffic accidents involving third parties.
Medical First Aid Record - Refers to the hospital treatment on the day of the accident (must include date, time, and reason for admission). It can be requested at the Social Service.

For each coverage, we list the mandatory and optional documents that help in the analysis.
Selfie Video of up to 1 minute reporting the events on the day of the accident, with date, time, and location.
Medical First Attendance Record - Referring to the hospital care on the day of the accident (must include date, time, and reason for admission). Can be requested from Social Services.
Exams and Reports - Requests and reports of all exams performed due to the accident.
Police Incident Report - Can be made at the police station after the accident (by declaratory act). It is mandatory for traffic accidents involving third parties.
Certificate from Fire Department, SAMU, or Highway Concessions - Must contain the signature of the responsible person, reason, date, and time of service.
Medical-Hospital Records - Record of the patient’s care history from the date of admission until discharge. Can be requested from Social Services.
Identity Card and CPF.
Proof of Residence - Water, electricity, gas, or internet bill from the current year.
Driver's License (CNH) - Must be valid.
Medical Report - Must inform the injury suffered and the treatment adopted, justifying the period of absence.
Medical Certificate - Must include the period of absence (with CID10), date, identification data of the doctor (full name, physical or digital signature, and CRM) and address of the hospital unit.
Bank Details - (Bank name, branch, and account) must be in the name of the insured, be an individual account, and be registered in the IZA app.
Proof of Income - Referring to the 3 months prior to the month of the accident, which may include:
Delivery workers - Report/extract of deliveries indicating monthly amounts received (can be the statement available in the delivery app).
Payslip;
Bank statement with identified receipts;
Service provision receipts (with payer's data, such as CPF/CNPJ and signature), along with the bank statement;
Income Tax Declaration.

Final Medical Discharge Certificate - Document with information on medical discharge after completion of outpatient follow-up.
Forensic Report from the Medical Legal Institute - Report from the IML quantifying and certifying the sequela of Permanent Disability after an examination.
In-Person Medical Report - Private medical report quantifying and certifying the sequela of Permanent Disability after an in-person examination. Must include date, signature, and stamp with CRM.
Guardianship Term (In case of mental incapacity) - Must be notarized.
Personal documents of the Guardian: ID, CPF, and proof of residence.
First Medical Care Record - Related to the care at the hospital on the day of the accident (must include date, time, and reason for admission). Can be requested from Social Services.
Police Report - Can be filed at the police station after the accident (as a declaratory act). It is mandatory for traffic accidents involving third parties.
Medical-Hospital History - A record of the patient's care history from the date of admission to the date of death. Can be requested from Social Services.
Certificate from the Fire Department, SAMU, or Highway Concessionaires - Must include the responsible party's signature, reason, date, and time of service.
ID Card and CPF of the protected person.
Proof of Residence of the protected person - Water, electricity, gas, or internet bill from the current year.
Driver's License (CNH) of the protected person - Must be valid.
Forensic Report of the accident site issued by the Criminalistics Institute.
Death Certificate of the protected person.
Bank Details of the Beneficiary(ies) - (Bank name, agency, and account) must be in the beneficiary's name and belong to an individual.
Death Certificate - Must contain information about the death of the protected person if it did not occur in the hospital. The document must include date, signature, and CRM stamp.
Autopsy Report from the Medical-Legal Institute (IML) - In case of violent or suspicious death.
Declaration of Sole Heirs - Declares the beneficiaries' relationship with the protected person. Must have 02 (two) witnesses, and all signatures must be notarized for AUTHENTICITY.
Documents of the Beneficiary(ies):
Spouse: Updated Marriage Certificate, ID, CPF, and proof of residence;
Partner: Stable Union Declaration (notarized statement), ID, CPF, and proof of residence;
Children: Birth Certificate, ID, CPF, and proof of residence.
Parents: ID, CPF, and proof of residence;

Identity Card and CPF of the protected person.
Death Certificate of the protected person.
Funeral Invoices - Proof of expenses related to the funeral of the protected person. It must include the date and a description of the services provided.
Identity Card and CPF of the person responsible for paying the funeral expenses.
Bank Details of the person responsible for paying the funeral expenses (Bank name, agency, and account) must be in the name of the account holder and be personal.
Term of Declaration and Release (if the funeral expenses were paid by third parties) - Declaration filled out by the beneficiary, informing payment of expenses by third parties, with notarized signature for AUTHENTICITY.
Documents of the person responsible for payment: ID, CPF, and bank details.
